How to Book an Appointment
Book Online
For your convenience, the quickest way to set up an appointment would be to book online. Select whether this is your First visit or a subsequent visit

1) Set-up a user profile using our online booking system; ‘JaneApp’
2) Please select “Initial Assessment”
3) Choose an available day/time that suits you & book it!
4) Please complete the patient intake & consent form that will be emailed to you prior to your appointment. Include details of any private health plan coverage you may have so that we can direct-bill on your behalf if possible

1) If you have not already done so, Set-up a user profile using our online booking system; ‘JaneApp’
2) Please select “Follow-up Visit”
3) Choose an available day/time that suits you & book it!
4) If you haven’t already done so, or it has been >6 weeks since your last completion, Please also complete the Patient-Specific Functional Scale (PSFS) form (here)

We can direct-bill most major providers on your behalf depending on your plan. There may be an excess or deductible remaining for you to pay.
The most convenient way for you to pay for your visit would be by pre-authorising us to bill your credit/debit card that you have uploaded into the secure ‘JaneApp’ booking system,
We also accept e-Transfer (payments@innov8physio.ca) and in-person payments through our POS terminal

We require notice by 3pm the day prior to your appointment to cancel or reschedule an appointment in order to avoid our ‘no-show/late cancellation’ fee. Please see the intake form for more information.

The Patient-Specific Functional Scale (PSFS)
At INNOV8 PHYSIOTHERAPY, we care about getting you back to what is important or meaningful to you.
Once you have booked your Initial Appointment, please take some time to complete our Patient-Specific Functional Scale (PSFS). This is a validated, patient reported measure that will help us to tailor your rehabilitation to what is most important to you.
